Condensation is the phase when water droplet form. Ex) When you have a glass of ice water the water droplets on the outside of the glass are condensation.
Evaporation is when a liquid heats up and turns into a steam or mist. Ex) When you boil water the steam that comes up from the pot is actually water!!
Therefore condensation is caused by cold temperatures, and evaporation is caused by hot temperatures.
